A. Contact the claims administrator. You can find the name and contact information on the patient’s dental benefit card.

Q. Where can I get a copy of the fee schedule?

A. Refer to your administration manual. These schedules are valid for the calendar year. Contact us for a current schedule.

Q. Do I use the same fee schedule for all Diversified Dental Services, Inc. patients?

A. Yes. The only difference is the actual benefits offered for the specific plan.

Q. How do I get updated materials for my administration manual?

A. You’ll receive periodic updates for your manual. If you’re missing materials, or aren’t sure if materials are current, contact us. 

Q. How can I verify which companies/clients are participating with Diversified Dental Services, Inc?

A   In your administration manual, you’ll find a list of clients affiliated with Diversified Dental Services, Inc. If you need an updated list, please contact us. For those in California, view the quick reference guide (PDF).

Q. How do I report an incorrect claims payment?

A. Call the claims administrator, indicate that you’re an in-network provider. If you’re unable to resolve the issue by directly contacting the administrator, contact us. 

Q. What is the National Provider Identifier Number (NPI)? How can I apply for one?

A. The National Provider Identifier (NPI) is a unique 10-digit identification number issued to health care providers in the United States by the Centers for Medicare and Medicaid Services (CMS).
