How to re-credential
After you’re one of our providers, you’ll need to periodically re-credential. We’ll send you a letter. Then follow these steps. Use the checklist (PDF) to keep track of what you need to send us.
- Complete the information form (PDF)
- Complete the application for re-credentialing (PDF)
- For additional locations, complete the Additional Location Addendum form (PDF).

How to change office information
If you need to update your office information, follow these steps.
