After you’re a part of the Diversified Dental Services network, we’ll help make your job easier. Keep us up to date on your practice, so our members can easily find you.

How to re-credential

After you’re one of our providers, you’ll need to periodically re-credential. We’ll send you a letter. Then follow these steps. Use the checklist (PDF) to keep track of what you need to send us.

  1. Complete the information form (PDF)
  2. Complete the application for re-credentialing for your state: Nevada (PDF) or Arizona, California, Mississippi and Utah (PDF)
  3. Return completed forms by mail or fax:

How to change office information

If you need to update your office information, follow these steps.

  1. Complete information update form (PDF)

How to cancel membership

If a dentist leaves the practice, or you no longer want to be part of our network, you’re required to notify us.
